Therapy Process Flowchart This document provides an overview of the process in requesting therapy support for your child at Fisher Road School
Working together Parents and therapists to read the document for an explantion of the process. This document outlines the aim of working together to enhance individual student learning. It defines the roles and responsibilities of our staff, parents/carers and therapists whilst engaged in the provision of therapy support.
To request therapy support
- Parents discuss with the class teacher -the goals of the therapy support must be in line with the students PDP
- Parent/carer request - Parents fill in the following, forward to the therapist and then submit to the school
- Please note the time and day MUST be discussed with the teacher prior to submitting the application.
- The request will be taken to the learning and support team for approval. An email will be sent to the therapist, parent and teacher after consideration.
Responsibilities for new therapist attending Fisher Road School:
1. Working with Children Check (PDF 74KB) complete declaration for child related work-specified volunteers and child related contractors or provide Department of Education no
2. proof of identity - 100 points please bring on first visit
3. Code of Conduct Policy please read
4. Controversial issues in schools policies and procedures please read
5. Child Protection Policy please read
6. Responding to allegations against employees policies and procedures please read
7. Allegations against Employees in the Area of Child Protection Policy please read
8. External provider agreement - must be completed by the agency, signed and brought to first visit
9. Child protection training - and provide a certificate or documentation that a suitable alternative has been completed
10. Provide evidence of Public liability, personal indemnity and workers compensation (or personal injury of a sole trader) insurance
All of this needs to be returned to the school before the learning and support team consider the request.
Responsibilities for a returning therapist to Fisher Road School
Ensure all insurances and certificates for training are up to date
